All volunteers must comply with the following requirements before being authorised to work in Lourdes.

DBS Certificates are valid for 4 years but the Volunteer Application, Safeguarding Self-Declaration Form, Code of Conduct and, where appropriate, Parental Consent Form must be renewed annually and can be downloaded from the links below.
 
Please note that if you already possess a DBS disclosure from another organisation you are still required to complete a form from Shrewsbury Diocese as we are a different registered body. If you are, or have been, the subject of any safeguarding concern or issue, or have any cautions, warnings or convictions please discuss these with Andrew O'Brien, Diocesan Safeguarding Co-ordinator, on 0151 652 9855 before submitting your booking. Having a criminal record will not necessarily bar you from work. Please refer to the Rehabilitation of Offenders legislation on the CSAS website. All details contained in a volunteer's DBS disclosure will be held in strict confidence by the Safeguarding Office. Failure to disclose any relevant information that is divulged on your DBS Certificate could result in your application being rejected.
