1. ONE BOOKING FORM is required for each person, including infants.
2. Please use black ink and write in BLOCK CAPITALS throughout. If you have difficulty writing, please ask someone to complete the form for you.
3. ALL boxes must be completed. The information requested is needed to enable your booking to be processed efficiently and to ensure your requirements are met so that your pilgrimage is as trouble free as possible.
4. Indicate your preferred hotel and second choice. If you wish to stay in the hospital then write HOSPITAL in this section. If sharing a room with a particular person/s please write their name/s in the space provided. Some single rooms are available at a supplement and triple bed and family rooms are sometimes available at no extra cost.
5. If you have special dietary requirements, eg vegetarian meals, these must be specified at the time of booking.
6. If you are receiving, or awaiting, ANY medical treatment you MUST answer the medical questions and complete the Preliminary Medical Information form.
7. If you have to fill in the Preliminary Medical Information form, please complete ALL sections, even if it repeats what you have already written on the booking form. It is better to exaggerate your condition as understating it may invalidate your insurance.
8. Limited 1st class rail travel is available at a supplement on a “first come, first served” basis.
9. Travel Only pilgrims must make your own arrangements for hotel accommodation in Lourdes and for getting from and to the station there.
10. Hotel Only pilgrims must make your own travel arrangements and take out your own insurance cover. If your arrival and departure dates differ from the main pilgrimage please give details in the Special Requirements box so that we can arrange with the hotel accordingly.
11. Coach departure points will be decided on when we know the numbers of pilgrims travelling from each area. Unless you are travelling with an organised parish group we will allocate you to the departure point nearest to your home, unless you specifically request otherwise, subject to seating availability. Pilgrims travelling by air to Lourdes are responsible for making their own arrangements for getting to and from Liverpool Airport.
12. If you are travelling as a member of a parish group or young peoples group please be sure to specify in the appropriate box.
13. Please write clearly on the Booking Form the amount of your deposit. The minimum is £50 but you can pay more if you wish. The balance may also be paid by instalments.
DO NOT SEND CASH.
14. If your fare is being paid by your parish SVP Society or a similar group please advise us and we will send the invoice directly to them.
15. All pilgrims are invited to offer their talents and gifts for the good of the pilgrimage.
16. The Sick Pilgrims Fund is available for use at the sole discretion of the Pilgrimage Director. Applications for grants from this fund must be made in writing to the Director and supported by a letter from the pilgrim’s parish priest. Applicants for grants will be informed by letter of the outcome of their application. Normally, a grant will be made only once to an individual and usually to pilgrims who stay in the hospital in Lourdes.
17. Allocations for places in the hospital will be decided by the Pilgrimage Doctors NOT BEFORE MAY 2002.
